MoMo Health Voucher

General Terms and Conditions

  1. By purchasing a MoMo Health Voucher (“Voucher”), you acknowledge that:

    1.  you have read, understood and are bound by these general terms and conditions (“Terms and Conditions”).

    2. the Voucher may only be redeemed through the specified service providers and in the manner as described in the product information available at www.prepaidhealth.co.za

    3. CareWorks will not share your personal information with any third parties other than those necessary for providing the services in respect of which a Voucher purchased relates (“Services”), or as otherwise required or allowed for by law.

    4. The Voucher bearer will be required to provide the Voucher number at the service/supply point for validation purposes prior to any goods or services being supplied in respect of that Voucher.

    5. CareWorks will not be held responsible for any cellular network service operator's delay in forwarding a Voucher to you.

    6. A Voucher may not be exchanged for cash or any other service/goods besides the actual service/goods purchased.

    7. CareWorks reserves the right to reject any Voucher that has been tampered with or altered in any way.

    8. CareWorks may use, store and otherwise process your personal information that is necessary for delivering the Services, including but not limited to health records, contact details, identifying details and purchasing records supplied to CareWorks by you and/or or any third-party service provider involved in delivering of the services, including but not limited to doctors, pharmacies and clinics.

    9. You may contact CareWorks on 021 673 5300 or at Block B, Regent Square Office Park, OB Doncaster Road , Kenilworth, Cape Town, to require access to, or the correction of, records of your personal information, or to object to the future processing of your personal information. CareWorks will treat all health information as confidential.

    10. A Voucher may not be used in conjunction with any other reward, voucher or discount.

    11. A Voucher is valid for 3 years from date of purchase, whereafter it automatically expires.

    12. Expired Vouchers will not be exchanged nor refunded.

    13. All Services bought with a Voucher are subject to price increases in line with annual medical inflation. In this event:

      1. you will receive notification 30 days prior to the price increase providing the new price and effective date of the price increase.

      2. After the price increase has taken effect, you will need to top-up unused Vouchers though your MoMo account.

      3. once the Voucher has been topped-up, the original Voucher will be replaced by the new (current) Voucher.

    14. No interest shall accrue on unused Vouchers.

    15. All services and goods are supplied by independent third-party contractors/suppliers and subject to the terms and conditions as specified by the contractor/supplier unless otherwise stated in the product description. The independent third-party contractors/suppliers are neither agents nor employees of CareWorks and none of their actions, whether negligent or intentional, can be attributed to CareWorks whatsoever and you accordingly hold CareWorks harmless in this regard. For the avoidance of doubt, CareWorks cannot be held liable for any damages of any nature that you may suffer as a result of using Services/goods supplied by third party contractors/suppliers and you accordingly hold CareWorks harmless in this regard.

    16. Vouchers are transferable unless otherwise specified in the product description. It is therefore your responsibility to ensure that the Voucher details are stored securely and not accessed by unauthorised third parties. Accordingly, CareWorks will not be held liable for a Voucher being redeemed by an unauthorised third party unless that unauthorised redemption was due to the negligence or wrongful conduct of CareWorks.

    17. CareWorks will not be liable to replace and/or re-issue a Voucher that has been lost, stolen, damaged or destroyed, unless same arose from the negligence and/or wrongful conduct of CareWorks.

    18. Misuse of the Voucher to create a misrepresentation to Service Providers and/or CareWorks is illegal and may constitute fraud leading to prosecution or other legal consequences.

    19. Service authorisation may be declined if you breach these terms and conditions.

    20. In the unlikely event that a product is withdrawn by CareWorks and the Voucher has not yet been redeemed, CareWorks will provide SMS notification thereof to you on your contact number linked to the Voucher purchase and provide a full refund in respect of the purchase price paid for that Voucher.

    21. Refunds will be provided to you only in the following circumstances:

      1. Under the circumstances contemplated in 1.1.20

      2. Where you request a refund within 5 (five) days of purchase of the Voucher and have not partially or fully used the services/goods

      3. Where the product design and specifications explicitly provide for partial refunds based on the partial utilization of the services.

    22. Refunds will not be provided where you, or any other person, has partially or fully utilised the product supplied other than where a partial refund is explicitly specified in the product design and specifications.

    23. Refunds will be settled into your MoMo Account.

    24. A refund may attract an administration charge to cover transaction fees paid and reasonable other administrative costs incurred.

    25. You will be notified via SMS on your contact number linked to the Voucher purchase confirming the refund as well as the amount refunded.

    26. Changes to the Terms and Conditions may be made from time to time and will be published on www.prepaidhealth.co.za

    27. These Terms and Conditions shall be governed by laws of the Republic of South Africa.

 

Nurse & Healthforce Video-Dr Voucher

  1. The Nurse & Healthforce Video-Dr Voucher may only be redeemed at a participating Dis-Chem or Medicare pharmacy clinic.

  2. Prior to purchasing the Nurse & Healthforce Video-Dr Voucher, it is your responsibility to consult the “Find a Clinic” function on www.prepaidhealth.co.za to establish the location and proximity of your nearest participating Service Provider. Failure to do so shall not entitle you to a refund of the purchased Nurse & Video-Dr Voucher.

  3. The Nurse & Healthforce Video-Dr Voucher may be used to access a pharmacy clinic Nurse, Healthforce Video-Dr as well as select acute medication prescribed by the attendant nurse or Healthforce Video-Dr and limited to a set Medicine List.

  4. Medicines supplied as part of the Nurse & Healthforce Video-Dr Voucher are subject to a limit, based on the total value as well as the Medicine List. An additional payment may be required in some circumstances. The Pharmacist will inform you of any additional co-payment needed before supplying the medicines. You may then choose whether to buy the relevant medicines.

  5. The Medicine List and corresponding limit referred to in 2.4 above, were designed to treat most common minor ailments. The Medicine List and corresponding limit may be changed without prior notice to you.

  6. A Nurse & Healthforce Video-Dr Voucher entitles you to one consultation with a Nurse and a Healthforce Video-Dr both as part of the same visit.

  7. A Nurse & Healthforce Video-Dr Voucher may be transferred to a third party. A Nurse & Healthforce Video-Dr Voucher may only be redeemed once by either you or the person that you have transferred it to.

  8. In the event that a Healthforce Video-Dr consult is not required either by you or the person to whom you transferred the Nurse & Healthforce Video-Dr Voucher, you will be refunded a portion of the purchase price of the Nurse & Healthforce Video-Dr Voucher into your MTN MoMo account as per the product description and specifications. You will receive SMS notification of the refund.

  9. CareWorks may, at its sole discretion, change the value of the refund as per paragraph 3.7 above in respect of unused Nurse & Healthforce Video-Dr Vouchers, after having provided you with 30 days’ notice via SMS of its intention to do so.

  10. Nurse & Healthforce Video-Dr Vouchers may only be purchased by individual natural persons and may not be bought in bulk by medical schemes, companies or other organizations.

    1. CareWorks has in place reasonable commercial standards of technology and operational security to protect all information provided by users from loss, misuse, alteration or destruction. All reasonable steps will be taken to secure a user's information. Authorised employees, who are responsible for the maintenance of any sensitive data submitted, are required to maintain the confidentiality of such data. The policy applies to all employees of CareWorks or public bodies that may receive such information from CareWorks. It is expressly prohibited for any person, business or entity to gain or attempt to gain unauthorised access to any page on this website, or to deliver or attempt to deliver any unauthorised, damaging or malicious code to this website. If a person delivers or attempts to deliver any unauthorised, damaging or malicious code to this website or attempts to gain unauthorised access to any page on this website, a criminal charge will be laid against that person, and, if CareWorks or any public body should suffer any damage or loss, civil damages will be claimed.
